2 year GENETIC Health Warranty – Purchase Agreement – Spay/Neuter Contract
This was written to protect both me as the seller and you as the buyer.

The Trupanion 30-day trial insurance coverage must be activated to make our warranties valid. This certificate must be activated the day before your scheduled pickup day. Proof of this activation must be shown before your puppy will be released to you. This warranty may not be available in all states, please check this before inquiring about our puppies. Any warranty claims in the first 30 days will need to be processed through Trupanion first.

The following conditions apply to the sale of the aforementioned puppy:

72 hour Well Puppy Warranty: All puppies are examined by a licensed Veterinarian and given a clean bill of health before they leave our care. To the best of the Breeder and Veterinarian’s knowledge, this puppy has no known diseases or illnesses and is guaranteed to be free of untreatable health conditions. Within 72 hours of possession the buyer has the option of having the puppy examined by their own Veterinarian and to personally inspect the dog. If, as a result of the examination, the veterinarian determines that the dog is not in good health or requires any treatment that the buyer does not want to provide, they may return the dog to the seller within 48 hours of the exam for a full refund of the purchase price. This well puppy warranty is void after the puppy is in his/her new home over 72 hours. If the buyer forgoes this 3 day well puppy warranty, they are then acknowledging that the
puppy was received in good health at the time of pickup.

Warranty Reimbursement Details:
All claims under the Genetic Health Warranty must be approved by Seller BEFORE any treatment is provided. Seller will reimburse Buyer for the reasonable and customary medical treatment necessary, after documented diagnosis, to correct or cure the condition or illness, up to the full purchase price of the puppy if documented to be genetic or hereditary in nature. Monetary reimbursement covers up to 100% of your purchase price. You do not have to return your original puppy to use this warranty. This warranty does not reimburse for any costs incurred before the diagnosis (diagnostic fees). We do not cover any elective proactive procedures.

Purchaser agrees to NEVER administer Trifexis, Comfortis, Bravecto or Nexgard to their dog regardless of veterinarian suggestion. This health warranty will be completely null and void should Dog be administered these agents which are reported to cause seizures, nervous system disorders, cardiovascular disorders, gastrointestinal disorders and or death.

Genetic Health Warranty – (2 years):
Ailments that qualify for reimbursement are those related to hip, elbow, eye, patella (knees) and heart only.
No other health conditions that may arise in the lifetime of the dog are covered by this warranty other than what’s described below. (Apple Creek Doodles will require access to all of the dogs’ vet records up to the point of illness to prove there were no injuries reported prior to the onset of symptoms.
Elbows, Heart, Patellas (knees) and Eyes: Heart issues must be confirmed by a Canine Cardiologist and found to be genetic in nature and require surgery, Eye issues must be confirmed by a Canine Ophthalmologist and found to be genetic in nature and require surgery, Patella issues must be confirmed by an Orthopedic specialist and require surgery and Elbow issues must be confirmed by O.F.A. x-rays and require surgery. We will reimburse surgery costs up to the full purchase price of the puppy once surgery is completed and documentation is provided to the Seller.

Hips: If you suspect your dog may have hip dysplasia Apple Creek Doodles must be notified immediately and before any procedures are done to make sure no unnecessary x-rays occur. There is a process and things we will require of you in order to confirm the diagnosis before we will issue any refunds. We will need copies of your dog’s vet records up to this point to verify there was no injury previously reported. A hip study from the O.F.A (Orthopedic Foundation for Animals) will be required, your dog must be sedated, digital x-rays taken, one copy sent to Apple Creek and the other sent to O.F.A for an official prelim hip report. This is the only documentation we accept to confirm the diagnosis of hip dysplasia. We will not honor a written statement or exams from your licensed vet, it must be documented by O.F.A and the results must be sent to Apple Creek Doodles. Official O.F.A rating of Moderate or Severe is issued a reimbursement of 1/2 of the purchase price if diagnosed within the 2-year warranty period. If surgery is required within the warranty period, we will reimburse the cost of the surgery not to exceed the full purchase price of the puppy once vet bills are provided as proof. O.F.A hip ratings of Borderline and Mild are not covered under our warranty (as they typically do not affect the quality of life of the animal) Apple Creek Doodles has the right to get a second opinion at the vet of our choosing.

Training, Temperament, Looks and Expectations: Buyer understands that all puppies exhibit certain undesirable behaviors (nipping, chewing, barking, jumping etc.) Buyer is responsible for appropriate training of puppy to correctly teach acceptable behaviors, and is responsible for proper socialization of puppy. No one can guarantee the future temperament of a dog due to environmental influences, improper training, lack of socialization, neglect or abuse, therefore we do not warrant such. Buyer acknowledges that there is no promise (express or implied) that their doodle will be non- shedding or non-allergenic. Buyer further agrees that the size of the dog/puppy and color are not predictable and no promise or warranty regarding size, color, coat appearance/texture, or any physical/behavioral trait is made by the Seller. Buyer is aware that Apple Creek Doodles is not responsible and offers no warranty or monetary reimbursements if their puppy does not pass training to become a service or therapy dog, we sell puppies as familycompanions only.

Spay/Neuter Agreement: Buyer certifies that he/she is not acting for another individual in the purchase of this dog. It is understood that the dog being sold to be a companion pet only and is not being sold with breeding rights. This dog must be spayed or neutered by 10 months of age, no matter your veterinarians recommendation. Failure to have this done will void the health warranty and will result in litigation to enforce this agreement. Proof of spaying/neutering by 10 months of age must accompany any request for warranty reimbursement. If this puppy is bred, the purchaser agrees to pay the seller five times the purchase price of this puppy for every litter produced.

Exclusions: Buyer is advised to discuss risk reduction strategies with their Veterinarian and to acquire private pet insurance as the seller offers nowarranty or guarantee of any kind for the above-mentioned issues. This contract does not include coverage for any illness due to the ingestion of foreign objects, food or chemicals, any physical injuries of any kind, any issues whose cause is unknown or may have an environmental link such as – Sarcomas, Tumors, Autoimmune Disorders, Thyroid Disease, Stomach issues, Food Sensitivities, Skin or Food Allergies and Seizures. We have no control of the area your pup may be in, food you choose to feed your pup, or the environmental factors that may trigger things to occur. Allergies in dogs (like people) can develop at any time. Seller has taken all reasonable care to produce pups that are as healthy as possible and free of any non-treatable illnesses. Conditions such as ear infections, eye infections, hernias, cosmetic teeth/bite issues such as an under-bite or base narrow canines, urinary tract infections, viral illnesses such as kennel cough, intestinal parasites such as worms, Coccidia and Giardia are these are considered common occurrences of pet ownership and are not covered under this warranty at any time. Dogs are living creatures, no one can guarantee they will be 100% perfect, by bringing an animal into your life you understand things happen and you are responsible for them both financially and emotionally for their entire life. We highly recommend purchasing Pet Insurance for your new puppy, having added coverage for things that may occur that are not our responsibility or covered in our warranty is a great way to further protect you and your new puppy. We are not responsible for any ailments that may be transmitted to any other pets as the puppy should be kept away from other pets until a negative fecal is confirmed and all core vaccines have been completed.

Full and Final Agreement: The parties agree that this writing represents the entire agreement between them and that no other representations, either oral or written have been made regarding the dog described above. Any agreement beyond this contract must be in writing, dated, agreed to, and signed by both the Buyer and Seller. All refunds are cumulative and once the full purchase price of the puppy has been refunded to buyer, for whatever cause, the warranties expire and Seller has no further monetary obligation to Buyer. By signing below, Buyer agrees that he/she understands and agrees with this contract fully, and that this contract is under the jurisdiction of the State of Michigan, in the county of St. Clair.

All reimbursements are limited up to the purchase price of the puppy no matter what the treatment is for or when it’s administered.
